• Photo ID
• Any Current X-Rays
• Completed forms

To properly & completely evaluate your dental need, we will refer to radiographs. If you have any current x-rays it will save you time and money to bring them with you. Current is considered 1 year for films called bitewings or PA's and 3 years for full months and panoramics.

You can also save time by printing the forms on this website (below) and filling them out before your arrival.
